Paediatric advance care planning
The PAC-Plan section comprises an introduction to the nature and purpose of advance planning in children, the PAC-Plan documentation itself and lastly the policy that supports it.

This film from together for short lives features interviews with two bereaved mothers – Josie and Julie – speaking about their experience of end-of-life planning with medical professionals, and is intended to raise awareness for professionals of some of the issues to consider when discussing end of life plans with families and promotes the powerful voice of the parent within this process.
PAC plan e-learning
We encourage all staff to undertake this elearning module which is linked to the NHS wales ESR.
